Your 1st Visit

What Should I do Before my First Appointment?

We request you fill out the patient forms under “For Patients” to dedicate all our time to your treatment. If you are unable to fill the forms out online, please arrive 15 minutes early in order to ensure your appointment begins at its start time.

What Happens During my First Appointment?

You will receive a very thorough evaluation for the first 20-30 minutes where measurements will be taken along with education regarding your pain and/or injury. Usually there is time for 15 minutes of manual therapy interventions (soft tissue massage, joint mobilizations, muscle energy) and an initial prescribed exercise program to take home.

What do I Need to Bring to my First Appointment?

  Please bring the following with you:

  • I.D. Card
  • Insurance Card (if applicable)
  • Referral from Physician (if applicable)
  • Form of Payment: cash, check, Visa or Mastercard, or HSA/FSA card

What Should I Wear to my First Appointment?

 Please wear athletic or loose-fitting clothing to all physical therapy appointments so we can easily treat the affected area while keeping you comfortable. For example, shorts are great for any leg issues, while tank tops are perfect for any arm symptoms. Good supportive shoes are required – no heels or open-toed footwear (sandals, flip flops).
